The overall goal of this position is to help individuals with spinal pain navigate the medical systems and receive timely and appropriate evaluation, treatment and education services. The patient needs may range from minimal to complex and may occur in the inpatient/ambulatory arena. The Coordinator is responsible for facilitating a patient and physician friendly experience within the Florida Hospital Spine Care Program. The Coordinator assists in planning and conducting community outreach activities, including screenings. Coordinator serves as primary intake and interviews patients who have spinal pain to obtain a patient history, demographic and insurance information. Coordinator assists patients in accessing appropriate resources as needed; directs and/or arranges appointments with providers according to a pre-established treatment algorithm. Coordinator receives phone calls, makes phone calls with prompt and courteous response from/to referring physicians/office staff, and patients, acting as coordinator for the patient and physician. Duties include scheduling consultations, requesting patient records, and assuring patients are registered. Coordinator may facilitate patient support and/or education groups and continually strive for innovative and creative strategies to impact and improve patient life quality. Coordinator will gather, record and evaluate program data and to use this data in establishing and evaluating program goals. Coordinator will facilitate, with a physician and nursing leader, an interdisciplinary clinical team to support clinical/operational goals, growth/volume/market goals and adherence to established patient pathways, etc. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

KNOWLEDGE AND SKILLS REQUIRED:

· CPR

· Skilled in providing age appropriate spine care education.

· Team-centered leadership and facilitation skills; ability to interact appropriately with teams to facilitate performance improvement; ability to interact with patients/families and Spine Care Program Team in regard to care management interventions and illness adjustment issues.

· Strong organizational skills

· Strong communication and customer relation skills; ability to interact patiently with patients with chronic pain and/or illness

PREFERRED KNOWLEDGE AND SKILLS:

· Data analysis

· Program development

· Case management

EDUCATION AND EXPERIENCE REQUIRED:

· Graduate of a school of nursing, physical or occupational therapy

· Three to five years experience in spine care

PREFERRED EDUCATION AND EXPERIENCE:

· Associate’s, bachelor’s or master’s in nursing

· Associate’s, bachelor’s and master’s in physical or occupational therapy

CVIII is a 16-bed surgical intensive care area servicing fresh vascular surgical patients and complex cardiac surgical patients. Diagnoses include, but are not limited to - triple A's, fem-pops, carotids, CABG, valve repairs and any thoracic surgical patients needing upper level care. The unit has a full-time Nurse Manager, four Assistant Nurse Managers covering days/nights, PCT assistance 24/7, and HUC coverage 7 a.m.-11p.m. Ratios are 1:1 or 1:2 depending upon the acuity of the patient. Educational support for staff is provided by a CV educator and Clinical Nurse Specialist. In addition, two pre-op educators are located on the unit to do bedside teaching for families and patients according to need. Staff has the opportunity to participate in the PPP (Professional Practice Program) similar to other hospitals with clinical ladders, train for relief charge duty and participate (if selected) to serve as preceptors and/or mentors for new nurses. The staff may also self-schedule. For nurses not trained in acute care, transitional education programs are available through Florida Hospital.




The CVIII Unit was built within the past 5 years and has the latest in construction features. CVIII is connected to Florida Hospital Orlando via a new breezeway that has a visitor waiting room and private consultation room for families. Rooms are all private with glass sliding doors for maximum viewing of patients. Computers and desk top charting areas are available in each room for nursing documentation purposes. In addition, staff has access to a large staff lounge and private locker room. Per Diem, Days/Nights shifts Available. Minimum 1 year recent surgical ICU or CVICU experience. Must have current Register Nurse Florida licensure.

On our 3 East- Surgical Unit we have 37 beds with private rooms. Nurses on this floor will typically care for patients who have had general surgeries ranging from abdominal to orthopedic as well as Medical overflow patients.


The nurse to patient ratio is 1:5 with assistance from patient care technicians. There is an Assistant Nurse Manager or Charge nurse available for each shift. The unit also has a trained Health Unit Coordinator to complete orders and answer phones for the nursing staff. The Florida Hospital Altamonte 3 East- Surgical unit is comprised of a caring multidisciplinary team of nurses, physicians and support staff to meet the needs of their patients.


Job Requirements:

•One year of recent Medical/Surgical experience required

•Basic Life Support (BLS) required

•Must be a graduate of a school of nursing

•Must be a Registered Nurse licensed in the state of Florida or actively pursuing reciprocity from another state.
