i. Flowsheet will be initially given to the NP by the admitting Transplant Coordinator with basic

patient info already filled in. Basic living donor info will be filled in by the Living Donor Coordinator ii. Daily labs will be downloaded and recorded by Lisa
iii. NP will record daily meds—immunosuppressants and antivirals
iv. Discharge meds list will come from Eliza’s final sheet that is usually faxed to Pharmacare or other discharge pharmacy. Eliza’s sheet can be attached to the flowsheet for Data Coordinator and clinic purposes.
v. Upon discharge, all of the dictations for the patient (including pertinent radiographic studies) will be printed by Lisa for inclusion in the patient’s clinic chart

c. Discharge teaching
i. Provide patient with Lab Order Sheet (FPL form) for first clinic visit’s labs—this will be made out by Lisa
ii. Call Lisa with the date of first clinic visit which will be decided upon by the rounding physicians

d. Discharge Clinical Resume Dictation

e. Maintain Transplant Readmission Form for patients that are readmitted post-transplant (even if Transplant Service is not consulted). Labs can be downloaded by Lisa.

f. Update Transplant database on H-drive (HanDbase program)

III. Q/A
a. Every other Monday @ Noon
b. List the new transplants for the previous 2 weeks
c. List the readmissions/complications for the previous 2 weeks

• PALS
