Job Description:
Registered Nurse-FL, Previous Home Care experience preferred
The Registered Nurse assesses, plans, implements, coordinates, monitors and evaluates home care services to meet the needs of the patient, caregiver and family members through communication, education and community resources to promote quality and cost effective outcomes within their home and community. When functioning as a case manager the registered nurse assumes responsibility for managing the patients care through to completion. Possesses basic computer skills. Demonstrates the ability to communicate effectively and possesses good organizational and time management skills. Displays flexibility, is self-directed and motivated. Ability to prioritize and follow responsibility through to completion.
