Job Description:
Care Coordinator will consult with patients in person, over the phone and/or web inquiries and
utilizing her medical skills and expertise refer patients to primary care and specialty physicians as
needed to address each patient’s specific health needs. Patients will come from front desk/E.D.
walk-ins, phone calls from business card distribution, hospital staff referral or physician practices.
Will provide a be a well-rounded resource for all Florida Hospital services and programs for patients and physician practices.

Will refer patients to community programs and/or social workers as needed for programs such as Shepherd’s Hope, Cuidate, PCAN clinics, American Breast Cancer Foundation, etc. Will act as a liaison for patients with scheduling and/or service issues with women’s services also be a liaison for physicians

Care Coordinator will represent her campus at marketing events to patients and physicians in the community becoming the “The Face of Women’s Services”. Promote WHP and FHEO physicians
Position as Women’s Partner in Health, create loyalty among women. Attends Physician Office Manager Luncheons, Healthy Women’s Society Luncheons, CHN Events when invited and all WHP and FHEO marketing events.
