JOB DESCRIPTION

Nurse Case Manager is responsible for coordinating efficient discharge planning and assisting with capacity solutions for Florida Hospital. Works with the multidisciplinary healthcare team to develop, activate and efficiently facilitate patient treatment and discharge plans. Has knowledge of Interqual Criteria for admission and continued stay.

· Knowledge of and ability to utilize inhouse and external resources.

· Flexibility in prioritization

· Ability to multitask

· Self motivated and able to work independently

· Knowledge of computer programs

PREFERRED KNOWLEDGE AND SKILLS:

· Experience in case management, managed care, long term care, home health care

· Experience in discharge planning

EDUCATION AND EXPERIENCE REQUIRED:

· LPN

PREFERRED EDUCATION AND EXPERIENCE:

· RN with BSN

· Health care experience

LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:

· For LPN or RN – Florida State Board of Nursing license
