Job Description:
The Case Manager is responsible for discharge planning with a goal of effective and efficient planning and coordinating the transition from the hospital to patient's post hospital environment. Identifies high risk patients through screening mechanism, performs assessment of patient and dtermines and facilitates the appropriate level of care and discharge needs. This position requires critical thinking and the ability to multitask. Experience in home health, nursing home and patient care is preferred.
