The overall goal of this position is to help individuals with spinal pain navigate the medical systems and receive timely and appropriate evaluation, treatment and education services. The patient needs may range from minimal to complex and may occur in the inpatient/ambulatory arena. The Coordinator is responsible for facilitating a patient and physician friendly experience within the Florida Hospital Spine Care Program. The Coordinator assists in planning and conducting community outreach activities, including screenings. Coordinator serves as primary intake and interviews patients who have spinal pain to obtain a patient history, demographic and insurance information. Coordinator assists patients in accessing appropriate resources as needed; directs and/or arranges appointments with providers according to a pre-established treatment algorithm. Coordinator receives phone calls, makes phone calls with prompt and courteous response from/to referring physicians/office staff, and patients, acting as coordinator for the patient and physician. Duties include scheduling consultations, requesting patient records, and assuring patients are registered. Coordinator may facilitate patient support and/or education groups and continually strive for innovative and creative strategies to impact and improve patient life quality. Coordinator will gather, record and evaluate program data and to use this data in establishing and evaluating program goals. Coordinator will facilitate, with a physician and nursing leader, an interdisciplinary clinical team to support clinical/operational goals, growth/volume/market goals and adherence to established patient pathways, etc. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

KNOWLEDGE AND SKILLS REQUIRED:

· CPR

· Skilled in providing age appropriate spine care education.

· Team-centered leadership and facilitation skills; ability to interact appropriately with teams to facilitate performance improvement; ability to interact with patients/families and Spine Care Program Team in regard to care management interventions and illness adjustment issues.

· Strong organizational skills

· Strong communication and customer relation skills; ability to interact patiently with patients with chronic pain and/or illness

PREFERRED KNOWLEDGE AND SKILLS:

· Data analysis

· Program development

· Case management

EDUCATION AND EXPERIENCE REQUIRED:

· Graduate of a school of nursing, physical or occupational therapy

· Three to five years experience in spine care

PREFERRED EDUCATION AND EXPERIENCE:

· Associate’s, bachelor’s or master’s in nursing

· Associate’s, bachelor’s and master’s in physical or occupational therapy
