GENERAL SUMMARY:
The Hospitalist Clinical Care Coordinator (HCCC), under the supervision of the Director of Hospitalist Program Services, functions in a supportive role to the Hospitalist Physicians, health care team, patients, and families to facilitate the delivery of quality, timely, and appropriate care. The HCCC is responsible for the assessment, planning, implementation, and evaluation of patients’ care needs to maximize the efficient utilization of acute care resources and promote early post acute care planning. The HCCC partners with the Hospitalist Physician in implementation of treatment plans, and works closely with the Case Management Team to provide input and facilitate services as needed. Actively participates in providing outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.

· Experience in Case Management, Managed Care, Utilization Management, Discharge Planning, and Home Health, preferred
· Experience in Critical care/Intensive Care/ED clinical settings a plus.
EDUCATION AND EXPERIENCE REQUIRED:
RN
Experience in acute care setting

· BSN
· Case Management Training and/or certification (CCM)
· Acute care experience with focus in ICU, PCU, or critical care areas
LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:

· Participates in daily rounds with Hospitalist Physicians including census reporting, discussion of treatment and aftercare plans, patient/family educational/instruction, and physician education for process improvement.
· Provides guidance to the Hospitalist Physician in determining appropriate level of care, utilization review criteria for admission,and continued stay, and discharge planning options. Utilizes critical thinking to review patients’clinical status using standardized clinical criteria per hospital policy.
· Ensures timely collection of statistical data, including audit requirements, payor source, quality performance indicators, and patient satisfaction for care received as expressed via the survey tools.
· Discusses discharge planning with physicians, nurses, case managers, other health care providers, and patients/families as needed. Incorporates age and developmental needs of the patient population into the discharge plan.
· Reviews the patient treatment plan for opportunities to facilitate efficient movement through the healthcare system and obtains clinical results and dictated reports to expedite care. Prioritizes work load to ensure assignments are completed.
· Communicates effectively utilizing all available internal and external methods, e.g., wireless, fax, voice mail, e-mail, etc. Documents pertinent data accurately in UR03, including clinical status summaries, and progress notes as indicated.
· Actively participates and cooperates with the Case Management Department in support of campus and system goals for capacity management and length of stay goals achievement. This includes, but is not limited to, identification of avoidable delays, initiation of treatment plans, identification of potential barriers to plans, attendance at Case Management Department meetings, educational presentations, and inservices.
· Maintains knowledge of federal and regulatory policies in the management of patient care.
· Attends professional meetings to participate in presenting findings to the Hospitalist Physicians, sharing feedback, and assisting with problem identification and resolution.
· Performs other duties as assigned or directed to ensure the smooth and successful operation of the department.

The RN is a registered professional nurse who is responsible for providing and supervising direct and indirect total nursing care responsibilities to identified age specific groups. Utilizing the nursing process (assessing, planning, implementing and evaluating) in achieving the goals of the nursing department. Adheres to the Florida Hospital Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies and accrediting bodies.
